    hello we are just finishing our final paperwork I-485 for fiance visa (k1) and had several questions wondering if anyone could help
    1 is the vaccination documentation worksheet the same as vaccination supplement or do we need to send this cd from st lukes or what?
    2 bipgraphic info - what address do they want for my wife(fiance) as now she lives here with me?
    3there are several other documents asked for on the I-485 do they need the originals or are copies ok (marriage cirt, fiance approval notice, vaccination supplement, form I-94, translation, and birth cirt,) any help would be greatly appriciated thank you

    1. You don't send the cd of your x-ray. Most people go to a civil surgeon in the US to get the immunization section (only) filled out of the form I-693 and sealed to submit. Do not let the CS tell you that you need a full medical exam. You can just send the vaccination supplement from her visa exam if it is marked with recent shots for MMR; Tdap/Td/DTP/DTaP (one of those); Varicella or a VH in the column beside it. Also the results section should say "incomplete...eligible for a blanket waiver" or "complete." and the doctor must have signed it. If her visa exam was durng flu season (Oct 1-March 31) then she would also need a flu shot on that vaccination sheet from her country. If all of those things aren't on her sheet then she needs the I-693 done in the US with those shots on it.

    2. This is a new process so her biographic information will include her new married name, her US address, marital status-married, and her full maiden name where it asks for other names used.

    3. Photocopies of everything are accepted. If you are scheduled for an interview, then take the originals with you to show them. Not all K1s have an interview. Some are processed in California and they mail you the green card after they look over your file.
