    Hello Everyone,

    I am a little confused with the form I-693. My husband is from the Philippines and completed his medical exam there. They only provided him his vaccination record and a CD-ROM. Is that what he should turn in with his application for adjustment of status?

    With his vaccination report DS-3025 if it says complete at the bottom then just a copy of this needs to be sent with AOS I-485 but if incomplete all you need to do is find a Civil surgeon in your area recommended by USCIS and have the vaccination supplement transferred over to the I-693 vaccination part only Part 1.2 and 5.

      The DS3025 does not have to say complete to be accepted by USCIS. It can say Incomplete AND Applicant may be eligible for blanket waiver if the required shots are marked on it. He should have

      1. Shots complete
      MMR
      Tdap or Td or DTP
      Varicella (or VH written in a column beside it if he had chickenpox)

      2. Results checked
      Incomplete
      Applicant may be eligible for blanket waiver

      3. Signed and dated by the doctor


      With those 3 things done, a photocopy of the DS3025 can be sent as proof of immunizations complete without seeing a civil surgeon. If even one thing is missing, then go to a civil surgeon.
