Regarding change of surname after marriage in case of H4 type

i have H1B visa for working in US. My wife has the passport before the marriage.
My questions are:
1) Her surname after marriage is changed. Now the new surname should be present in the current passport or not required.
2) The marital status is changed after marriage then the new marital status should be present in the current passport or not required.
3) Do we need to apply for change the passport or the older one works fine.

Please help.
