lost of h4 visa

I applied for my H4 visa extension this Sep 2012, but instead got H1B approval notice. So, my hubby inquired his office to re-check.
Then I was informed that my status has been changed to H4 again but I din't get the changed status papers.
Now when we need the H4 status papers, they are lost.
Presently, I've the H1B approval notice. What can I do? Somebody give suggestions...

Initially it was USCIS's mistake, instead of filing for H4 they did for H1, and we informed them thro our attorney, and they changed it to H4.
But the papers are lost somewhere between attorney/my hubby's office. Could'nt trace out?
Can I re-apply now? How long will it take to get my H4?
