H1B - DS160 Query

Hi All,

While filling DS160 I encountered following questions:

Are you applying in the same country where the visa above was issued and is this country your principal country of residence?
I had h1b visa which was expired in 2011. I travelled on that visa and today I have applied for h1b. Currently I am in India. So the answer to this question in NO. Is it correct?

Also there is question regarding your previous visa:
Have you been ten printed? => I really don't remember about that. I got my visa in October 2008. What will happen if I say 'YES'?
