H1 extension ater 6yrs with PERM

Hi

I was actually on H1B visa and i applied PERM LABOR when i was in middle of 6th year of H1B. My Labor went to audit and i am unable to get H1B extension. I moved to H4 Visa as dependent. But now my labor and I-140 got approved while i am on H4.

1) Am i eligible for H1B renewal with the approved I-140?
2) can i get extension in US iteself without going to India?
