Lost passport and I-94

Hello,

I have lost my passport and entry I-94(white card) on my way to Indian consulate which will expire 15th of next month. I have a H1 visa stamp in that passport until Dec 2006. Now I am in the process of applying for a new passport but now my question is

1) Do I need to apply I-102 for I-94(US entry White card, When departing the U.S., all visitors are required to relinquish their I-94 cards) ???

Is I-102 for H1 I-129 that comes with 1-94 at the bottom or the departing I-94 white card?

2) Can I apply for US visa in Canada/Mexico??

I **** can't belive this...LOL

I see no one replied to my questions but anyways I don't give a **** now as I have found my passport. I wish good luck to those who got into same problem as I did.
