H1B still shows Initial Review

My H1B was accepted on 8th April'13 however the status still shows 'Initial Review'

Questions:

1. Normally the H1B effective date is 1st Oct, is it possible that this can be delayed since my H1B still shows initial review?

2. If I get an RFE then how much time will it take to get an approval?

3. I am currently on H4, after H1B approval if I do not get a job within 15 days then do I need to file for H4 again?

Hello, Saxena.

Normally, there is ample time provided to file for an RFE response.

But, isn't an H1B filed BY an employer? You mentioned not finding a job after getting H1B. Who is filing for this H1B, then?
