H1b Extension vs Transfer and Extension at the sametime

Hi,

I'm working for a company A where they have filed an H1B extension (I-129) which is still under initial stage my current H1B is going to expire in the month of Dec '13.

I want to join a new company and wanted to know where the new company would need to initiate the H1B Transfer and Extension? If they do, do they need to do it in Premium Processing?

Also what all cons does this have when I try to transfer and do the extension when another extension is underway?
