Lost Passport and I-94

Hi,

I lost my Indian passport with I-94 slip in it. I hold a valid H1B visa. When I landed in USA, I was given a I-94 slip. But the data is now available in US Customs and Border protection website.

I registered police complaint and I will have to re-apply for lost passport in Indian Embassy.

I have few questions:

1. Do I need to re-apply for lost I-94 or else can I carry the printed copy of I-94 when travelling back to India??
2. I have complaint number but I don't have hard copy of police compliant. Is that required for applying Indain Passport in Embassy??
3. Any idea how long would the process take??
