VISA payment fee receipt

Hi,

I have below questions regarding VISA stamping. Please some one provide answers for it.

I changed my employer after 10 months and now I am going India for vacation.

1. I paid my VISA fee using IMPS service through online banking. I don't have fee receipt with me. Please let me know if fee receipt is needed even if you pay through online banking.
2. One of my friend told me that passport size photo is no more required for VISA stamping.
3. I got one more I94 after VISA transfer. Do i need to attach it with my original I94 and submit both I94 while leaving US.
