Effective date of LCA v/s H1B start date

Hi,

Can anybody point me to the right source of information or provide an answer for below situation and question

Situation: H1B extension is approved and is valid from Jan 2014. LCA filed as part of this H1B extension is valid from Nov 2013. End dates of H1B and LCA validity are same.
LCA has revised wage rate compared to LCA on previous H1 petition (before extension)

1. Is employed required to pay new LCA wage rate from H1B extension start date or from LCA validity start date?

New wage should be effective after expiration of the old H1B validity period.
