My visa approved on Oct 2006 and I stayed in USA till Feb 2009 and applied for first extension and it got approved. But I have not used extension for stamping and not travel back to USA.
Am I eligible for applying an CAP exemption coverage?

You would be eligible for Cap exemption and remaining time till Feb 2015, 6 Years from your last held valid H1 status.
