Newborn Baby's Indian Passport...Please advise

Hello Friends,
Here is the situation, please advise.

1. I am in US with my wife name (maiden name) endorse in my passport
2. my son is Indian citizen and has birth certificate with my wife's name (married name) as his mother
3. We do have marriage certificate (maiden name)
4. I do have NOC from Indian consulate in USA with wife name (maiden name) on it.
5. I am trying to get an affidavit stating maiden & married name person is same
6. Also I do have annexure H with my wife's (maiden name) as she is applying the son's passport

Please let me know, if anyone had similar situation and will my wife face any issues in getting passport for my son in India....
