Query regarding DS156, 221(g), H1B

I attended interview at the consulate. However the consulate officer asked me to bring additional documents and gave me 221(g) and asked me to come for interview again. Now I have got the documents and I am planning to go for the interview. My question is do I need to fill the DS156 again or is it okay if I take the same copy of the previous DS156? Note that earlier I filled it online, so I got a barcode. If I have to fill again, then in the section "Have you ever been refused a US Visa?", should I mark it as "Yes" or "No"?

Yea, we need to mark it as "Yes" in the section "Have you ever been refused a US Visa?".
