Dependant Visa stamping

Hi,
I have my H1 B Visa stamped in Septermber 2014. But i didnot travel to USA and still in India.
Recenly got married and now my wife is going for H1 B dependant Visa with US consulate.
Please clarify

1. Does she need to carry my original Passport along with her for the visa interview? Or Color photocopy of my passport will do?

2. Does she need to carry last 3 months of my USA payslips. I dont have USA payslips as didnot travel and start working for the employer yet.
