H4 EAD Application form filing questions

Hi All,

I am filing I-765 for my wife H4 EAD, confused in Q16
what should we put here , I read the FAQ but did not get answer , if some one know please post here.

Also Which USCIS office we need to send this ?

The USCIS is expected to publish instructions and FAQs about the application process for H-4 EAD. You cannot apply for the EAD till May 26 and we should have more information from USCIS before that.

Thanks , do you know what set of documents we require except below,

1. Approval copy I-140
2. All past 797's
3. Spouse passport copy
