Need Advice on H1b and H4

My wife and I we are planning to take up an appointment for H4 (for Wife) and for H1B (For me). Do I have to take 2 DDS for 4,600 and 2 DDS for 276 or do I have to take one for 4,600 and other for 276 for my appointment and my wife’s appointment?
Do I have to take the appointment only for me and my wife can accompany me or we both needs to have separate appointments? Appreciate your earliest response.
