H4 EAD - Quick question regarding "maiden name"

I came to US 4 years ago after marriage. I had change my last name before I entered the US and also made a new passport with my husband's last name.

My question is regarding form I765 - there is a question regarding "other names used (include maden name)". Since from day 1, I have been in US with my husband's last name, should I still enter my maiden name here? Or just mark the field as N/A? If entering my "maiden name" is required, will my old passport copy work?

Please advise.

The question is not about whether you have used any other name since entering the US, it is about ever in your life.

Therefore, you would mention your maiden name (name before marriage) in other names field.
