I entered the USA in Jul 2013 and I got the I-94 with expiration date of Sep 2015. Later, I got the new H1B petition approved "I-797A" (changed my company here in US) with extension of stay with new I-94 till Dec 2015.The I-94 copy (Dec 2015) is attached in the new approved I797 copy (at the bottom). However, the online I-94 is still showing the old date, that is Sep 2015. (https://i94.cbp.dhs.gov/I94/request.html)

Do I need to take any action to get this corrected or I don't have any action item to take as new I-94 will get issued on re-entry (if I travel outside country and get my Visa stamped).

You don't need to take any action. When you re-enter again, your will get a new I-94 at the POE (With a different A number) which will be listed as another entry in your travel history.
