Questions on Form I-539 - H1 to H4 with I-140 Approved

Hello,

I came to the US in 2006 on L1b visa. Moved to H1b visa in Oct 2010 and have my I-140 approved with a priority date of July 2011.
Due to some personal reasons, I need to move to H4 on my husbands H1 visa for some time. I am filling the I-539 for the change of status.
Could someone please tell me what do I need to fill in for the following questions since I have my I-140 approved? Thanks much in advance.

Has an immigrant petition EVER been filed for you or for any other person included in this application?
