H4B Visa for My Family

My family is going for visa stamping to India. I have few questions:

1] In Passport Details while taking appointment : Do I need to fill my Wife's details?
2] What should be the Principle Applicant Name : It should be my name / wife name?
3] Petitioner's Name: My name / Company Name?

It is your wife's application, you are only helping her fill it. So, answer accordingly.

Principal should be you and petitioner should be your company.
