    Please answer the below questions.

    1. In other names section, can I use nick names & other unofficial names?

    2. My name in passport is 'AAA BBB CCC DDD'. But in some educational documents, my name is 'AAABBB CCCDDD'. So, in other names section, can I give the latter?

    3. In 'Date and place of birth' section in personal page, there is a field for 'State/Province'. This detail is not mentioned in my passport. Should I fill this field?

    4. Last year, my Visa application was rejected in lottery. Should I fill 'Have you ever been refused a U.S. visa' as 'Yes'?

    5. If I am entering my colleague as contact person, what should I enter in 'Relationship to You' field?

    1. In other names section, can I use nick names & other unofficial names?
    >>> Nope. That field should be used only if you have an official name change.

    2. My name in passport is 'AAA BBB CCC DDD'. But in some educational documents, my name is 'AAABBB CCCDDD'. So, in other names section, can I give the latter?
    >>> Go as per the name in the passport.

    3. In 'Date and place of birth' section in personal page, there is a field for 'State/Province'. This detail is not mentioned in my passport. Should I fill this field?
    >>> In DS160, YES. Do not write anything in passport.

    4. Last year, my Visa application was rejected in lottery. Should I fill 'Have you ever been refused a U.S. visa' as 'Yes'?
    >>> That is not visa application. That is called H1B petition. Visa and petition are two different things. If you have never filled a DS160 to get a U.S visa and if your visa has never been refused by a U.S consulate, then answer NO.

    5. If I am entering my colleague as contact person, what should I enter in 'Relationship to You' field?
    >>> Colleague. If he/she is your supervisor, then enter that.
