Advice on Amendment

Hi experts,

Need your advice on below scenario

Situation:
I have my H1B confirmed.
Then later I have been promoted to a higher role in India and hence also an upgrade in my roles and responsibilities at onsite role as well. Now I believe this would make it obligatory (or not) for me to get the amendment done.

Query:
1.Since I have my H1B, can I travel and then apply for amendment after I reach onsite? Is it mandatory to complete the amendment and only then travel?
2. Do I need H1B and LCA both amended?
3. What is the current trend for processing time for amendment?
4. What woudl be the processing time for premium?
