Receive New I-797 after H1b Amendment ?

Hi

My H1B petition is valid until Sep 2016.
I got 1 year approval for the current H1B petition.
And still 5 years left in my H1B.

If i apply for H1B amendment, will i receive a new I-797?
If so, would that new I-797 may be issued for 3 years? (If all the documentation is fine)

What is the amendment for? Amendment can be filed with or without an extension request.
