H-4 extension I-539

Hello,
I need to file for my spouse's H-4 extension I-539 and I have some doubts.

My current status:
Currently on H-1B expiring in Sept 2016,
Approved I-140 (EB3),
Employer already filed for H-1B extension (Ist extension) and received I-129 receipt from USCIS.

Spouse current status:
Spouse is on H-4 visa expiring in Sept 2016

Doubts on I-159 form:

Part 4: Question#3: Are you, or any other person included on the application, an applicant for an immigrant visa? Yes or No

Based on our current immigration situation explained above, what should be the answer?

Part 4: Question#4: Has an immigrant petition EVER been filed for you or for any other person included in this application? Yes or No

Based on our current immigration situation explained above, what should be the answer?

General Question: At what address I need to send my application. Is it same where my H1B extension has been filed?
