H1B Amendment Approved for a last year date (expired date)

Hi All,

I have received my amendment approved on May 20th 2016 ( filed on Spet 2015 with my previous VISA which was valid till Sept 2015) and the validity in approval is only till Sept 2015 .
At the time of my amendment filing i had extension in progress which got approved in Feb 2016 & the validity till Dec 2017 .

Does this mean that the latest amendment is my valid date and i am out of status? Or should i file my amendment again ?

Please help me to figure out this issue.
