H4 EAD - I-94 docs

Hi

I am filing for H4 EAD for my wife. When we entered USA, I-94 is stamped till Jun'17. After that I apply for an extension, H1 and H4 got extended till Jun'20.
As part of that extension I have received a copy of I797, which at the bottom has I-94.

So should i be sending this I-94 which is the bottom copy of latest I-797 that shows till Jun'20
or should i download from the government website https://i94.cbp.dhs.gov , but this only shows till Jun'17
or should i send both

Any help is greatly appreciated.

The one at the bottom of your latest I797, which shows the current expiry date of your visa viz., June 2020.
