H1B Premium extension approved but H4 is still pending

Hi All,

I applied for H1B extension initially on Jun 2016 and got a RFE. While replying to the RFE, I updated my case to premium processing beginning of feb. My extension was approved on 2/14 but my wife's case is still pending from June 2nd. It's been almost 16 days from the time my case was approved. I have also not received by I797A from USCIS.

Two questions:-

a. Is this normal?
b. Since my H1B premium was approved, will I be receiving the I797A/I129 earlier or I have to wait till H4 for my wife is approved and they will send the document together?

H1B Premium extension approved but H4 is still pending

I am in similar situation. My H1 and H4 extension was filed in normal processing on Aug 26 2016. I converted my H1 to premium on March 1st 2017 and got approval for my H1 on March 10th. But my wife's H4 is still pending.

Question:
1. My Wife's I-94 expired on 9/24, does that 240 days limit is applicable for dependents?
2. If my H1 Extension is approved till 2019, can my wife stay in US past 240 days, if we don't get an H4 approval by then.
3. Is there something I can do at this point to get the decision on H4.

If someone can please answer these questions that would be great.
