Travel with with Vias on old passport & Renewed Passport?

Hi,
Could you please let me know whether it would be any problem
i.)have valid h4 visa valid for next 2 yrs.
ii.)passport got renewed and got new passport but the valid visa is in the old passport.

will there be any problem while entry into US? which passport no. should be mentioned while filling the form at port of entry?

Staple your old and new passports together. Mention both passport numbers indicating whichever is the new one.
