H1B Renewal

I need your help on H1B Renewal (Cap or No-Cap)

I stayed in the USA from 2007 until 2013 (completed 6 years). Filed extension with I-140 approval.

My H1B petition got expired on 03/03/2015. I'm in India since 2013.

Since my I-140 approved (2010) and the H1 was valid till 2015, will I come under exempt? or should I require fresh H1?

Is there any timeline to apply for renewal (for example: six years from the H1b expiry)?
