How do I correct typographical error and replace lost EAD card

Hi all,

I received my H4EAD in Feb 2018.
The EAD had a typographical error due to USCIS error. So, I sent the EAD back to USCIS Nebraska Service Center. Unfortunately, the EAD is misdelivered and is lost.

What should I do to correct typographical error and also replace lost EAD card.
