Urgent!! H1B transfer Question

I would appreciate if someone can provide me accurate information about following scenario.

Company A files my H1B. My application is in the process and meanwhile I get an offer from Company B to join them. Also during all these developments, H1B quota gets over. In such a case, can Company file for H1B visa transfer even when I have not yet recieved any decision on my application?

I shall really appreciate a prompt reply on this issue.

Once the approval relized in the name of company A, it will not get transfer to any other company till you go to US...

I am already in U.S. so what in tht case? Also, once I rcv approval on my aaplication which was filed by Company A, then can Company B file for my H1 visa transfer immediately?

Another question:

H1 visa filed by Company X gets approved but becomes effective only in december. During this period I work on my Practical traning with company X. Suppose before december I get a better offer from Company Y, then can company Y file for my H1 visa transfer before december (when my H1 kicks in)??
