H4 Visa - Name incorrect on I-797A and I-94 forms

My wife's H4 visa was approved in 2016 along with my H1 (VISA is is expiring in May 2019). Her name was incorrect in I797 and I94 forms. Her first name is in given name and given name is in first name (probably due to the error I made when filling the application).

Can someone tell me the procedure to correct the name? Thanks in Advance.
