Urgent question regarding H4

I am on h1B visa since oct 2005. I am getting married in this dec. I had couple of questions.
1. For appointment after getting HDFC reciept does she need other documents other than DS156 and 157 like approval notice etc right now or at the time of interview is fine?

2. Is it advisable to get an appointment right now or wait until marriage is done so that passport has her new surname and middlename?

3. If she can get the appointment right now what does she fill out in surname and middle name colomn? Also in the address colomn as well.

4. Do I need to add her name in my pasport as well or its ok without her name?

Visa and Passport

---- Yes, your would be wife does need DS 156 and 157. If she is in India she does'nt need any approval notice. A copy of your H1 approval notice will suffice. She has to present the forms and other documents at the time of interview, but print it out a week before else you may loose the opportunity to fill the forms as the site will close on your application (check the vfs website for this)

2. Is it advisable to get an appointment right now or wait until marriage is done so that passport has her new surname and middlename?

---- Ideally, she has to wait till you get married, get a new Passport with the marriage certificate and then apply for the appointment.

You can also seek an appointment now by paying the fees on the present passport and later at the time of interview present the marriage certificate, marriage photos and if she has a new Passport (In which case she has to defend herself)

---- Whatever her present surname is and which is on the Passport

---- If possible get it added, but remember that you will be issued a new Passport for that
