Question on DS 156

Hi Gurus,

I have a question on DS-156 form Question 18 -

It says spouse's full name (Even if divorced or separated. Include maiden name.)
My wife's name in the passport is <FIRSTNAME> <LASTNAME after marriage>. The Lastname is same as my last name.

Do I have to enter in this field as <FIRSTNAME> <Lastname before marriage> <LASTNAME after marriage> (since it says include maiden name) or in the format as is in her passport?

I think the name should be exactly as in passport.
