Hi,
I am on H1B visa. I just took an appointment online from https://www.vfs-usa.co.in (Mumbai consulate) for my future wife who is in India.
At the end of application, there was a checklist which says all the documents to be send before 5 days of appointment. One of the document required was "Additional Questionnaire in original". I cannot find that document when I took the appointment. They only gave me pdf's link to DS156, DS157, appointment letter, guidlines and checklist.
DO I need to send "Additional Questionnaire in original" for H4B visa also. If so where can I find that document
