H1B Interview Queries

I’m going to H1B stamping interview and wanted to know couple of quick questions about same:

I was in US on H1B VISA around 5 years back and that time I applied for Green Card but after that I had to come back to India. I never continued after that for Green card and never filed for Form I-485 etc.

Now here are my questions:
1. In form 156 (which we have to submit for H1B stamping) there is column:
36. Has Anyone Ever Filed an Immigrant Visa Petition on Your Behalf?
I wanted to know what I need to fill here.

2. Is it mandatory to declare if you have any relative (Husband/Wife, Father/Mother, Fiancé / Fiancée, Son/Daughter Brother/Sister)? Can we hide that information? Is there anyway consulate people can trace that out?

Reply Yes for that question if I-140 was filed for.

Please do not hide the relevant information. For H1 you are allowed to have immigrant intent.

They may not find out right away but it is still a fraud with serious consequences. Telling the truth will not.
