If someone is currently in the US working on the basis of a H-1B visa and needs to extend it for the first time, which application would need to be submitted? And does this have to be done in the US or can it be done on their behalf in their home country (e.g. South Africa)?

You need to first get H1 status extended through an employer. Procedure is just like initial H1.
If you need to get H1 visa stamp extended, you have to do in your home country, or you can do in Canada/Mexico if you have never been out of status.Immihelp Support
