H1 and Fellowship

I am a physician currently in my third year of Residency on H1. I got a fellowship(3 more years) again on H1. My residency and fellowship will consume 6 year of H1. I am interested in doing one more year of sub-specialization after that. What are my options. Suggestions are appreciated!
