I would like to apply for my H1b Extension.. for another 3 yrs. I got approval on 01/01/2004 to 01/01/2007 - 1st approval. and i would like to apply for my 1st extension... so what are the documents i should produce to my employer and how much fee i have to pay to INS... is $190 + $1500 + $500. please guide me

And also if its for 2nd extension how much i have to pay - filing fee... to INS
