error while creating a new application

Respected Sir/madam,

I am trying to create a new non-immi visa application (H4) when i put the info incluidng barcode it keeps saying Cast from string "undefined/undefined/undefined" to type 'Date' is not valid. I would like to know what it is. I am putting the correct date still this message is unchanged.
