H4 Stamp Corrections

Hi,
My wife lost week attend for depend visa (H4) in Telugu in Chennai consulate. It seems she filled Nationality wrongly as Iranee instead of India in Form 156. So the same was replicated in her stamp in Passport.
Also, she got visa validity 11th May,2008 but my H1B is valid still October, 2009.

Please point us to the right directions to get these corrections.

contact consulate for directions, why did she not point out the issue at the time of interview
