help regarding DS-156 form

Hi,
I was answering the DS-156 form to send it to visa office for my H1 visa revalidation and stuck with the following questions, and can anybody help me by answering the following questions,

1. Question 19 of DS-156 says "What is the purpose of your trip? "
My question is, I am already in US, and I am just sending this form to visa office for revalidation, so, do I need to just leave this blank or do I need to fill up this with any particular information ?

2. Similarly, Question 20 of DS-156 : "At what address will you stay in the US" ? do I need to give my current address ?

3. Question 21 of DS-156, When do you intend to arrive in the US ?
4. Question 22 of DS-156, how long do you intend to stay in the US ?
5. Question 23 who will pay for the trip?
6. Question 24, persons travelling with you ?

Finally, DO I really need to answer the above ones ? help is appreciated.

1. Work for sponsored employer.
2. Yes.
3. Already in US.
4. Date of H1 visa expiry.
5. Self through employment.
6. If you have wife/children on H4 with you, write there names.

I heard from a friend that, nowadays it takes somewhere between 5-8 weeks for revalidation

I mailed my passport for revalidation to sate department, it is been 5 weeks now. Is there a way we could find out the status from the state department regrding our case???
