I am presently working abroad (Other than US), I may be relieved from my present duties by 8th June. I am planning for visa stamping interview (H1B and H4) at Mumbai consulate on 24th June, In Form DS-156, column 20 and 21 (Name and address of present employer / present occupation), which details I should mention? Details about the last employer would be fine?
In Form DS-156, column 8 and 9 (Other Surname used / Other First and Middle name used), married woman should write which details?
In Form DS-156, column 33 (Do you intend to study in the US), my son will be study in US, but at present I am not sure about the school name and address, after selecting “Yes”, what should we mention below?
Does additional questionnaire required for all applicants (H1B & H4) or for principal applicant (H1B) only?
