H1 approval after employment gap

I had H1 approved after the few weeks of employment gap. I completed with 4 years of H1.

(a) With new I-797 I didn't get I-94 so do I have to travel India immediately to get stamped?

(b) Do I need to take an emergency appointment with VFS? Please let me know the procedure

If you visa has expired, you need to get a visa first and then re-enter with the approved H1 I-797.

I don't know about Emergency appointment part.
