H4 extension

I changed my employer and have a new H1 but my wife visa H4 expired from my last employer. I need to extend her H4 visa without leaving US, Can someone help me and provide info like:

1. what form I need to submit (eg: I-159 or OF-156? or ..)
2. where(address) I should be sending the application to?
3. what documents I need to provide to INS for her visa extension.

H4's should be extended even before they expire, by applying to INS.

Now what you can do is send your and your wife's passports to state dept (St.Louis) and revalidate visas so that you all can travel and come back into USA. For that you can goto US state department web site and click on revalidating visas.
