Need help urgent - H1 revalidation

I would greatly appreciate it if somebody could answer my query.
I had sent my passport for revalidation to St. Louis. I just received it back without the stamp. They need another $50.
My question is - Can I just send them the passports with the payment or do I need to fill out another application, employment letter etc.?

Please help.
