Information regarding dependants Urgent

Hi all

I took visa interview appointment date for me n my daughter she is 2yrs old.
My question is only my name is being displayed in the appointment confirmation hers name is not displayed.Does her name has to be displayed
on the appointment confirmation letter.
Do i have to fill ds-156 for her also.
Did i make a mistake in taking the appointment.
