H4 to H1 and multiple I94s

Thanks for reading this post. Experts, I need your advice on my issue mentioned below.

I have a question regarding H1B, H4 and I-94 status.

I am currently in US on H4 which is getting expired Sept 30th. I applied for H1b from 2 companies. The company A has received the H1B approval papers with I-94 (COS) effective from Oct 1st. If I do not join this company, do I still need to get approval letter and I-94 from them?

I am planning to apply for H4 extension in the mean time so that I can be in H4 status. In case if I get I94 for H4 extension and since I already have I94 from company A, is it required for me to surrender all I94s?

In case, if the company refuses to give me the I94, what status I will be in (H4 or H1)? what options do I have?

The company B is asking me to wait for the approval letter from USCIS. I can't work for company B until the approval is received by company B.

Is it required for me to surrender all the I-94s when I leave US ?

Also, I would like to know if company refuses to give me I-94 which they have already have, how can I get a copy of the same ?
